　この部分は私が使用しているプロバイダなので、「白」と判断したのですが、
　私が使用しているプロバイダ中に不良会員がいると判断すべきなのでしょうか？

ちなみに上記部分は

ヘッダ（とスパムの本文）を全文貼り付けて，プロバイダに「同じ
プロバイダからスパムが届きました．　大変不愉快なのでヘッダと
本文を見て，該当するメール送信者に警告してください」とメール
してみてはどうでしょうか？

どうやら、昔の＠niftyのメールサーバと同じ問題を抱えているようですね。
送信者のHELOのパラメータの文字列は記録されますが、IPアドレスが記録されないので、追跡ができないようです。

TCE氏が指摘してくれたように、
お使いのISPのサーバの設定に問題がある可能性があります。
ただし私としては上のヘッダからだけでは100％確信が持てず、
おっしゃるように同じISPを利用したspammerの可能性も若干在ります。
（感覚的にはほとんどない気がしますが．．．）

　上の状況を確信するには、山田様に届いている他のメール
すなわち御友人からのメールなどに関してもいくつか解析してみて、
発信者の情報がきちんと残されているか、それらでも確認することです。

　掲示板上に公開して下さっても良いですが、真っ当なメールのヘッダを
載せることに抵抗があるなら、私・高崎まで、
真っ当なメールのヘッダをいくつか送って下さい。

　直接メールで送っていただいたヘッダを拝見したのですが、
やはり送り出したサーバのIPアドレスを
受け手側、すなわちjanis.or.jpさんの方で
きちんと残さないような設定になってしまっているようです。

　そこのネット業者様がどのような意図でそうしているか分かりませんが
ユーザのセキュリティを考えると、決して勧められない設定です。
スパムに限らず、各種のメールによる悪意的な行為に対して
発信者への対処が遅れたり、
またネット業者の設定によっては追求できない可能性があります。

　今までこのような場合には、そのユーザの方に、
利用なさっているネット業者に改善を依頼するよう勧めてきました。
ほとんどの場合で対処が行われたような気がします。

　一番大きいところでは、niftyの場合がありました。

　niftyの場合にはそれなりの理由があったように推察していますが

ネット業者によっては単なる設定ミスであり、
